Social Work
At Memorial Health, social workers are available to assess the unique situations of patients who are in the hospital or who see Memorial Care providers. Our goal is to help patients and families navigate difficulties and manage stress that may be affecting the patient’s physical and emotional well-being.
We can help you and your loved ones by:
- Providing information on advance directives or other legal documents that spell out your wishes if you become seriously injured or ill
- Helping your healthcare providers understand your unique needs
- Helping you and your loved ones understand the care you are undergoing and the roles of each of your healthcare providers
- Helping you and your loved ones with healthcare navigation
- Connecting you with services in the community that offer financial assistance for medications, medical equipment, medical bills and other expenses
- Assuming responsibility for the guardianship process
- Providing patient and family support and advocating for their best interests
