Traditional Medicaid
Traditional Medicaid is a medical assistance program funded by the state and federal governments. If you are covered under this program through Illinois Healthcare and Family Services (HFS), please provide an eligibility card for each appointment of service received at a Memorial Health affiliate. For more information, please visit Illinois Healthcare and Family Services. Note that all Memorial Health affiliates accept traditional Medicaid coverage.
Medicaid Managed Care
On April 1, 2018, the State of Illinois adopted a new Medicaid Managed Care program called HealthChoice Illinois. This program is available to Medicaid patients in all counties within Illinois, provided they qualify for coverage based upon State of Illinois requirements. Please consult our Insurance page for insurance coverage options at each MH affiliate. If you are covered under HealthChoice Illinois, please bring your insurance card to each appointment.
To change or choose a Medicaid Managed Care health plan, visit EnrollHFS.Illinois.gov and click “Enroll.” If you have questions or want to enroll by phone, call the State of Illinois Client Enrollment Services at 877-912-8880 .
Note that Medicaid Managed Care enrollment information is available through EnrollHFS.Illinois.gov. Patients currently covered under this program will receive communication directly from the Illinois Healthcare and Family Services department regarding the re-enrollment period. Within the Memorial Health primary service area, the enrollment period is set to occur at or just after the beginning of each calendar year. We encourage patients within this program to pay close attention to their mail for re-enrollment information.
