Community Health Collaborative

The Community Health Collaborative at Lincoln Memorial Hospital connects healthcare providers, organizations, businesses, private individuals and churches to improve the health of the people and communities it serves.

Our Vision

The vision of the Collaborative is to create the healthiest community in the nation.

Inspired by the Centers of Disease Control “Four Considerations to Improve Health and Well-Being for All,” the LMH Community Health Collaborative has four objectives:

  1. Expand and enhance access to clinical care
  2. Promote and encourage healthy behaviors
  3. Improve the physical environment
  4. Address the social determinants of health.

The Collaborative Advisory Board

The LMH Community Health Collaborative is guided with assistance from an Advisory Board composed of leaders across the community who represent nonprofits, businesses, education, healthcare, government, philanthropists, faith-based organizations and public health. The LMH Community Health Collaborative is funded by and operates under the approval of the Lincoln Memorial Foundation. A board member of the Lincoln Memorial Foundation serves as chair of the LMH Community Health Collaborative Advisory Board. Administrative leadership is provided by the LMH Community Health Director.

The Collaborative Network

The Community Health Collaborative network offers information-sharing and networking. Anyone interested in creating the best quality of life possible for Logan County residents is welcome to join the LMH Community Health Collaborative. The collaborative meets quarterly. Coalitions report to the network and address priorities established in the community health needs assessment or as needed.

The Work of the Collaborative

We address the needs of our community through partnership recognizing that the most meaningful impact on health and well-being for all is through a shared vision and collective impact. The Collaborative recognizes that an unending list of needs exists. A community health needs assessment is conducted every three years to determine the local health priorities to determine focused efforts. The projects chosen will fall into one of four action areas: socioeconomic factors, physical environment, health behaviors and clinical care. We recognize that a project in one of these areas may produce positive outcomes in another.

Whether an individual project or a long-term issue, the Collaborative invites critical players to the table to efficiently use all local resources and take action on important issues affecting Logan County residents. The collaborative network meets approximately four times per year. Committees may be established to facilitate the objectives of the Collaborative and meet as needed.
