Population Health Services

Memorial Health provides a number of population health services designed to achieve optimal health and to reduce overall healthcare spend. Our teams work with your healthy employees to connect them with the preventive services needed to maintain health, as well your rising- and high-risk employee populations to create better health outcomes and reduce total costs of care. The services available to you include the following:

  • Pre-visit planning: In collaboration with your employees’ providers, we work to identify and close care gaps to ensure all your employee preventive care, health maintenance and wellness needs are met.
  • Referral management: Our referral management team collaborates with providers to keep appropriate care within our network, taking this burden from the employee and managing total cost of care for both you and your employees.
  • Care Coordination and Population Care Management: Our Care Coordination team specializes in working with employees with complex medical challenges, such as asthma, diabetes, cardiac-related needs and other high-risk health conditions. We collaborate with each employee and their provider to target, achieve and maintain positive health outcomes and appropriate healthcare spending.
  • Palliative Care: For employees whose healthcare situations are more complex and unable to be remedied by care coordination alone, our Palliative Care team focuses on addressing the pain, symptoms and stresses faced by employees experiencing serious illness so employees are empowered to live higher-quality, more satisfying lives while reducing overall healthcare expenditures.
  • Nurse Concierge: Led by experienced nurse practitioners, our nurse concierge phone line works with employees to answer questions and connect them to the care they need, when they need it.
  • Data Analytics and Reporting: As a partner in your employees’ health, you will receive monthly custom reports on your employee health outcomes enabled through Memorial’s proprietary population health management system. This system uses intelligent analytics, based on your program goals, to measure progress toward desired outcomes.
  • Primary Care with Guaranteed Access at All Memorial Sites: As a part of this partnership, your employees will receive enhanced access and services via 21 existing sites of care at Memorial Care and Decatur Memorial Medical Group locations.

Health and Wellness Programs

As a part of your plan services, Memorial Health Partners will include health and wellness initiatives designed to engage your employees in healthy initiatives. These include interaction with wellness coaches and dietitians, personalized training sessions and group classes, wellness fairs, weight loss programs, diabetes management and prevention services, as well as a number of other initiatives to sustain overall health.

  • Wellness Programs: MH offers programs such as wellness fairs, Tobacco-Free Me coaching, certified diabetes education and prevention, weight loss and healthy living programs and virtual cooking classes, among others.
  • Health Risk Assessments and Individualized Health Coaching: All employees will have the opportunity to complete a health risk assessment and biometrics, which will be used to identify potential or existing health risks and to partner employees with an appropriate health coach to set and achieve wellness-based goals.
  • Weight Loss Programs: Our weight loss programs offer employees access to experts in their fields who will partner with patients in activities designed to achieve employees’ weight loss goals, such as one-on-one meal planning, monitored exercise programs and support groups.
  • Virtual Symptom Screener: Our virtual symptom screener allows you to track employee health through a simple series of questions designed to identify COVID-19 symptoms before employees come to work, allowing you to follow up with appropriate care plans while preventing the spread of COVID-19 in the workplace.

Employee Assistance Program

Memorial Behavioral Health (MBH) offers employees a comprehensive Employee Assistance Program available to all employees (even those not on the health plan) and dependents. This benefit allows for employees and their families to access needed assistance at no cost to them when facing substance use disorders, career-related issues, relationship issues, personal stressors and other challenges.

  • Memorial Health Employee Assistance Program Call Center: Memorial Choice EAP will provide a 24/7 call center to provide your employees with brief assessment, intake and scheduling services.
  • Individual/Family Counseling Services: We will provide your employees, their spouses and dependent children services via video, telephone or face-to-face encounters (post-pandemic). Areas of service include relationship or family concerns, personal or emotional concerns (including grief), behavioral health diagnoses, stress, illness, career-related issues, trauma and substance abuse.
  • Wellness Consultation: Memorial Choice EAP offers employees, their spouses and dependents a free wellness consultation on topics such as health condition management, creating healthy routines, physical activity, nutrition and accountability to wellness goals.
  • Workplace Critical Incident Response. Memorial Choice EAP offers critical incident response during times of need, providing colleague assistance from professionally-trained clinicians after a loss, traumatic event or significant change to the workplace that negatively impacts colleagues.
  • Advance Care Planning. Our Advance Care Planning team works with colleagues, spouses and loved ones to plan for future medical care at no cost to them. This includes choosing treatment options in the event of a serious diagnosis, defining treatment values and wishes, selecting a healthcare agent, understanding life-sustaining treatments and putting these plans in writing. We also offer complimentary services to review and update these advance directives.
  • Eldercare Assessment. Memorial Choice EAP offers consultations for aging loved ones regarding their medical history, physical and cognitive status, medication regimen, living environment and financial and legal concerns. Provided by experienced, licensed professionals, these services help colleagues and loved ones find solutions to live independently or determine potential assisted living situations.
  • Employer Education. Employer education opportunities presents actionable discussions on themes designed to promote healthy lifestyle choices and participant self-awareness. Topics include work-life balance, self-care in the workplace, stress management, resilience, trauma-informed care, preventing and managing burnout, workplace wellness and healthy teams.
  • Data Reporting. Quarterly reporting will reflect key metrics selected by the employer. Data will protect member confidentiality.

Virtual Care

Memorial Choice virtual care ensures your employees receive the care they need, even when they are unable to meet with a care provider in person. Additionally, unlimited and timely access to their health records keeps your employees up-to-date and informed on their health status and care.

  • Online Appointment Scheduling: MH offers online appointment scheduling to employees through the Memorial Health App, allowing employees to set needed appointments with MPS providers at any time from their phone or computer.
  • Urgent Care Virtual Care: Urgent Care has two convenient care options for employees: Walk-in and On My Way. These options allow for employees to find their nearest urgent care location and see how many people are waiting in line at that time or check in before leaving home to reserve their space in line.
  • Health Biometrics and Records Access: Memorial’s Check My Health questionnaire provides employees with a snapshot of their current health, assisting in setting health goals and strategies for improvement. Additionally, the Follow My Health patient portal allows employees to access their health records, view test results, renew prescriptions, communicate with their care providers and more.
