What Every Worker Should Know About Unemployment Insurance

Time-Off Benefits

Q.  Will my unused paid time off (PTO) balance be paid to me when I leave?
A.  All unused PTO will be paid in your final paycheck.  The amount paid will be calculated by multiplying the total PTO hours in your bank by your final rate of pay.  Please note that this amount is subject to taxation.  Prior to termination, you can check your current PTO balance by logging into Colleague Self Service (CSS) and choosing the Personal Information / Time Off Balances link. If you have questions on your final paycheck, please contact the Payroll Department at 217-788-4913.

Q.  Will my unused sick time balance be paid to me when I leave?   
A.  Because sick time is intended to be used only when a colleague is unable to work due to their own illness or injury, sick balances are forfeited upon termination. 

Q.  May I take PTO time in lieu of my resignation notice period or to extend my termination date?  
A.  PTO time may not be used to extend employment after last day worked. 


Medical, Dental, Vision, Life, Disability and Flexible Spending Accounts

Q.  When will my medical, dental and vision benefits end?
A.  Coverage for medical, dental and vision benefits will terminate on the last day of the month in which your employment ends.  

Q.  Can I continue my medical, dental and vision benefits after I leave MH?
A.  You may extend your medical, dental and/or vision coverage through COBRA for up to 18 months after your coverage ends.  Information about COBRA and how to elect continuation of coverage will be sent from The Taben Group to your home address on file.  You must complete the COBRA election form and return it to The Taben Group within the time frame requested. 

Q.  How much will my premiums be under COBRA? 
A.  Current monthly COBRA rates are as follows:

Q.  When does my participation in the flexible spending accounts end?  
A.  Your participation in the Healthcare Flexible Spending Account and Dependent Care Flexible Spending Account ends on your termination date.  Eligible expenses incurred prior to the date participation ends may be reimbursed by filing a claim with Navia Benefits within 90 days. You may elect to continue participation in the Healthcare Flexible Spending Account through COBRA for the remainder of the plan year.  Information about COBRA continuation will be sent from The Taben Group to your home address on file following your termination. COBRA continuation is not available for the Dependent Care Flexible Spending Account.

Q.  When does my life insurance benefit end?  
A.  Coverage for eligible full-time and part-time colleagues under the MH paid basic or colleague paid voluntary supplemental Group Term Life Insurance Plan will end on your termination date.  You have the option to continue this coverage as an individual plan.  You will receive information from MetLife mailed to your address on file about conversion or portability options that are available to you.

Q.  What happens to my long-term disability coverage?
A.  Long-term disability coverage for full-time colleagues ends on the date of termination.  There is no conversion option available for this plan.  

Q. What happens to the voluntary benefit plans I elected to enroll in?
A.  If you enrolled in:

  • Critical illness, hospital indemnity, accident insurance through Hartford, please visit The Hartford to download and complete the required forms.  Forms must be returned to Hartford within 90 days following your termination.
  • Whole life insurance through Voya, please call 800-537-5024 within 30 days to request the necessary documents.
  • Allstate Identity Theft Protection, please call 800-789-2720.

Retirement Benefits

Q.  How can I access my retirement benefits?
A.  MH Defined Contribution Retirement Savings Plan:  Once you receive your final paycheck, you will be entitled to receive the value of your vested account. A termination packet will be mailed to your home address by Fidelity Investments after your termination date. Questions may be directed to Fidelity at 800-343-0860403(b) accounts:  Contact your individual investment provider for information on plan rollovers and/or withdrawals.

Other Important Information

Q: How do I provide feedback regarding my employment with MH? 
A. To talk to an HR representative, please contact the Colleague Relations team at 217-788-3580. You may also receive an email with a link to a brief and confidential exit survey. We ask that you take the time to complete it as a way of providing valuable feedback.

Q.  How can I change my address if I move after I leave employment?  
A.  Please email address changes to HRRecords@mhsil.com or call the MH Records team at 217-788-3580.

Q.  How will I receive my final pay from Memorial?  
A.  Your final pay will be issued to you with the same direct deposit allocations as your regular paychecks. Since your access to CSS will be disabled, a copy of your final direct deposit advice will be mailed to the address on file as of your termination date. Direct deposit allocations will be terminated after your final paycheck is processed.

Q.  How do I receive copies of previous pay information after I terminate my employment with Memorial?
A.  You may contact the Payroll department at 217-788-4913 to obtain previous copies of pay information.  Be sure to verify the address on file with the payroll representative.

Q.  How will I receive my tax documents?
A.  Your tax documents will be accessed by visiting w2.mhsil.com and creating an account and following the steps found in the user guide.

Important Note: You will not be able to register on the tax document site until your W2 has been loaded by Memorial Health. W2s will be loaded no later than
Jan. 31.

Q.  What happens if I had an education grant?

A.   If you leave employment prior to completing the work commitment contained in your education grant, you will be responsible for repaying a prorated amount of the total education grant award. For example, if you were awarded a grant of $10,500.00 with a three-year work commitment but you leave the organization after two years, you must repay $3,500.00. Repayment is determined by calculating your hours worked from the start date of the work commitment through your separation date from the health system. Q.  What happens if I had a sign-on or retention bonus?

A.   If you leave employment prior to completing the work commitment as outlined in your agreement, you will be responsible for repaying the full bonus amount. This can be done in one lump sum or in monthly payments with 10% interest. If you have any questions regarding your sign-on or retention bonus, please contact Colleague Relations at 217-788-3580.

Q.   How do I notify someone of activities by MH or any of its affiliates, colleagues, contractors, vendors or medical staff members that are unethical or illegal?

A.  Call the compliance hotline at 1-800-541-9331
