Healthy Jacksonville uses a community health model to increase access to healthcare for vulnerable and at-risk individuals. Community health workers work with individuals and families on deficiencies in one or more of 18 self-sufficiency measures. The goal is to help clients develop a medical home for their healthcare and better manage their personal and family health and wellness.
Also, Healthy Jacksonville works with community health workers to develop and strengthen relationships with community partners. As access issues are identified, these community partnerships are invaluable to help clients receive targeted assistance.
Healthy Communities Collaborative
The Healthy Communities Collaborative, an extension of the Healthy Jacksonville program, connects social service providers who provide access areas where individuals struggle with self-sufficiency. The Collaborative is divided into four workgroups that address the highest needs identified by clients in the Healthy Jacksonville program:
- Income/Employment
- Food/Nutrition
- Transportation
- Mental Illness/Substance Use Disorder
Each group takes on projects to improve access in their respective areas. The Healthy Communities Collaborative will develop tools for case managers to use to assist their clients as they navigate access to key assistance programs.
