Memorial Health's mission is to improve the health of the people and communities we serve. If you or a loved one had an amazing experience by a Memorial Health colleague, we want to know!

The Colleague of the Month program offers monthly winners a surprise celebration reception, a special presentation and thanks by a member of the executive leadership team, and a variety of other recognition incentives for living the values of Memorial Health.

Colleague Nomination

Please submit only one colleague per nomination form. Submissions that include multiple nominees will be considered ineligible and will not be reviewed. If you wish to recognize more than one individual, kindly complete a separate nomination form for each person.
