Memorial Care Locations Recognized as Patient-Centered Medical Homes

The majority of Memorial Care primary care locations have been recognized as Patient-Centered Medical Homes by the National Committee for Quality Assistance (NCQA).

The designation recognizes Memorial Care for using evidence-based, patient-centered processes. The medical home is accountable for meeting patients’ physical and mental health needs, including prevention and wellness, acute care and chronic care. Following guideline-directed care, the team coordinates care across all services of a healthcare system, including specialists, hospital, home health and community care.

This designation is awarded as a result of our commitment to providing our patients with the best care possible.
