Decatur Memorial Foundation Community Grant
Purpose
The Decatur Memorial Foundation awards grants to central Illinois organizations with 501(c)(3) classifications and educational institutions in communities served by Decatur Memorial Hospital. 2022 grants will be awarded for programs that align with the following focus areas:
- Increasing access to health care
- Addressing economic disparities
- Mental health (which includes substance abuse)
- Healthcare Education
- Agriculture
Grant applications are reviewed by the Grant Review Committee, which includes members of the Decatur Memorial Foundation Board of Directors, Foundation staff, and representatives of Decatur Memorial Hospital. Grants are awarded as funding permits and to those who meet qualifications.
Grant Awardee Responsibilities
Within 12 months of the grant award date, all grant recipients must submit a grant closeout report which includes grant expense reconciliation. If the amount of the grant award is greater than the amount of documented expenses, the awardee is required to reimburse the Decatur Memorial Foundation at the end of the 12-month grant period. Failure to submit final reports may result in ineligibility for future grant funding.
The Decatur Memorial Foundation requests that grantees use the following language in any publication, presentation or marketing associated with the approved project. “This project is powered by a grant from the Decatur Memorial Foundation.”
The 2023 grant cycle is not currently open.

Decatur Memorial Foundation Agriculture Scholarship
Two $5,000 scholarships are awarded each year to high school graduates who have selected a career in agriculture at a two or four year college. Applicants must be a high school senior or undergraduate. The Decatur Memorial Foundation and Board of Directors administers these scholarships.
Materials required for application
- Completed & Signed application
- Transcript from High school
- Two letters of recommendation
- Proof of enrollment (Copy of acceptance letter or copy of fall class schedule)

Dr. Thomas W. Samuels Nursing Scholarship
One $2,500 scholarship is awarded each year to a deserving student who is pursuing a nursing degree at Richland Community College. Applicants must be a high school senior or already enrolled in Richland’s Nursing Program. The Decatur Memorial Foundation and Board of Directors administers these scholarships.

- Two letters of recommendation

Decatur Memorial Hospital Auxiliary Healthcare Support Professionals Scholarship
Three $5,000 scholarships are awarded each year to high school graduates who are pursuing a career at a two or four year college in a healthcare support profession (i.e. nursing, x-ray tech, lab tech, pharmacy tech, etc.). The Decatur Memorial Hospital Auxiliary administers these scholarships.

- Two letters of recommendation
- Proof of enrollment (copy of acceptance letter or copy of fall class schedule)
